Step 1:

請填寫申請表，於規定時間送交承辦人辦理。
Please complete the application form and submit it within the specified period.

1. Applicants with a paper/poster/oral presentation: Form1
2. Applicants without a paper/poster/oral presentation: Form2

Step 2:

本案經學生事務暨學術發展委員會討論後，經承辦人通知，請至學務處下載推薦表及個資收集告知函，填寫完畢盡速送交承辦人。

After review by the Student Affairs and Academic Development Committee, the responsible staff will notify you.

Please then download the Recommendation Form and the Personal Information Collection Notice from the Office of Student Affairs, complete them, and submit them to the responsible staff as soon as possible.
